The NJ 2-1-1 Resource Database includes health and human services, government assistance programs and local community resources located in New Jersey. If your organization satisfies our inclusion standards, please submit your data.
Our new Resource Database Providor Portal now enables you to update your existing listing online.
If you already have an Agency Sign-In ID you can update your listing as often as is needed by going to the Resource Database Provider Portal to sign in.
If you do not have an Agency Sign-in ID, you will need to request one the first time you enter the Resource Database Provider Portal. When we receive this request, our data content manager will send you an electronic copy of all your agency programs and services currently listed in the database and your agency log-in infomation. Complete the Provider Form. Please Note: All forms must be completed in order to properly list your services. If you have several services, please find additional new service forms here. Remember to save the service form for each service.
Save all forms to your computer.
Email them to NJ 2-1-1 at firstname.lastname@example.org; OR fax them to us at (973) 887-4680; OR mail them to: NJ 2-1-1 Partnership, PO Box 346, East Hanover, NJ 07936.
The information you provide will be accessible to the general public who access the database from this website and will be used by our resource specialists who respond to inquiries from callers who are looking for assistance.
