Jersey Assistance for Community Caregiving (JACC)

Service Description:

A state program for seniors, age 60 and older, who require a nursing facility level of care but wish to remain at home. Services include homemaker and home health care, case management, respite care, home modification, special medical equipment and supplies, chore services, personal emergency response system, adult day care, and transportation. Recipients must be ineligible for Medicaid and Medicaid waiver programs and have resources at or below $40,000 for an individual or $60,000 for a couple. Participants share in the cost of their care on a sliding scale based on their income.

Eligibility:

Open to individuals in need of nursing level care, but who are ineligible for Medicaid. For single applicants, income resources must be at or below $40,000. For couple, the income must be at or below $60,000.
