Service Description:

The Your Way Home Call Center is the single point of entry into Montgomery County's new system for centralized screening, assessment and referral for homeless services.

The Call Center will quickly assess the vulnerability and determine service prioritization (i.e., prevention, rapid re-housing or permanent supportive housing) for Montgomery County residents who are homeless or at-risk for homelessness using an appropriate screening tool. The Call Center will then either schedule families and individuals for an in-person assessment by a Housing Stability Coach at one of the County's new community-based Housing Resource Centers, or refer them to another appropriate service provider.


All calls are free and confidential.


Open to Montgomery County residents who are homeless or at-risk for homelessness.
