Middlesex County Multi-Assistance Program (MCMAP)

Service Description:

The Middlesex County Multi-Assistive Program (MCMAP) provides cost-share assistance for homebound frail/elderly ages 65+, and disabled adults ages 18-64, who are ineligible for other assistance programs, but are in need of services to help them remain safely in the community. This "safety net" program was primarily established to help prevent premature nursing home placement. Care managers arrange for and ensure provision of appropriate services based on a care plan developed with the client.

Assistance can include:

- Prescription Assistance (up to $1500 annually)
- Home health care (up to 6 hours/week)
- Adult day program (up to 2 days/week)
- Medical adult day care (up to 1 day/week)
- Home delivered meals (1 meal per weekday)
- Barrier-free home modifications ($4500 lifetime maximum)
- Durable medical supplies and equipment (up to $600 annually)

Medical equipment and supplies includes items such as diapers, colostomy bags, diabetic supplies, canes, crutches, walkers, commodes, bath chairs and similar products not covered by Medicare.


Sliding scale fees. All clients must pay a cost share based on their income.


Must meet all of the following criteria:

- Middlesex County resident
- Homebound frail/elderly age 65+, OR age 18-64 and disabled
- Must NOT be on Medicaid or any other public assistance program (except for meal clients)
- Income and assets cannot exceed $50,000
