Enables offers in-home personal care services for adults and children with disabilities and senior citizens who need assistance with daily living tasks. Services include:
- Community based supports
- Behavioral supports
- Representative payee services
- Community inclusion
- Natural supports training
Care management for senior citizens is also offered.
Grant-funded and self-pay options are available. Please call for information.Eligibility:
Open to individuals with a disability or seniors who are living on their own or with their families. Applicants may be adults, children, or seniors, single, married couples, or parents with children.
Enable assists persons who have permanent or chronic disabilities, as well as those who are elderly and those whose disability may be the result of an injury or of a medical condition such as multiple sclerosis, stroke or other debilitating illness. The disability may be physical or cognitive and it causes difficulties with communication, self-care, mobility or the ability to live independently.
There are no age restrictions for services. Enable does not provide service to persons whose primary disability is mental illness or substance abuse.Application Instructions:
