Supports Coordination - In Home Supportive Services

Service Description:

In Home Supportive Services helps adults who have a disability that lasts for over a year stay in their own homes, go to work, attend school, and participate more fully in the community.

Services include:
- A dedicated Supports Coordinator will help individuals create a personalized care plan
- Supports Coordinators will provide consumers with ongoing support, reevaluating service plans and hours of care as needed
- In-home visits are scheduled at dates/times of the consumer's choice


Services may be free for elgible individuals. For others, a co-pay may be assessed.


Program participants must be adults, ages 18 to 59, mentally alert, have a physical disability that will last for at least one year, and reside in Philadelphia, Bucks, Montgomery, Delaware, or Chester County. Services are also available to residents of all other counties in Pennsylvania.
