Service Description:

Pennsylvania Department of Aging (PDA) Waiver Program provides home and community-based long-term care services as an alternative to nursing home care. After a thorough assessment to determine eligibility for the program, an assigned care manager will design a care plan with the client and monitor appropriate services.

If a client is determined to be eligible, the following services may be available at no cost:

- Homemaker Services Home-delivered Meals
- Mental Health Counseling Emergency Response Systems
- Caregiver Respite Services Medical Supplies & Equipment
- Home Environment Modifications Personal Care
- Speech Therapy Home Health Aide
- Adult Day Care Registered Nurse
- Transportation

Once enrolled in the PDA Waiver Program, clients are assigned a care manager. A care plan is developed jointly by the client and the care manager, after which services are authorized. The client will then be able to choose any company or agency from a list of participating providers.

Eligibility:

There is financial eligibility criteria. Senior citizens, ages 60 and older.
