Community Care Program for the Elderly & Disabled - Freehold

Service Description:

Community Care Program for the Elderly and Disabled (CCPED) is a federal home and community based service program designed to help individuals remain in the community or return to the community rather than be cared for in a long term care facility. This medical assistance program offers limited covered services including:

- Home health services
- Adult day care
- Non-emergency medical transportation
- Case management
- Homemaker assistance
- Prescription assistance
- Respite care

Also part of this program is the Caregiver Assistance Program (CAP) and the Assisted Living/Alternate Family Care program, for individuals who need additional care in order to remain in the communty.




Open to individuals eligible for nursing home level of care in Monmouth County. Applicants must be between the ages of 65 or older or between 18 and 64 and determined to be disabled by SSA or the Division of Medical Assistance and Health Services.
