Service Description:

Federally Qualified Health Center (FQHC) that provides free and reduced fee health care to the pediatric, adult, and geriatric homeless of Atlantic County, New Jersey. Services include:

- Prenatal and perinatal services
- Cancer and other disease screening
- Well and sick child services
- Immunizations
- Screening for elevated blood lead level
- Screening for communicable diseases and cholesterol
- Screening for eye diseases
- Hearing screening
- Screenings for preventable dental conditions
- Disease state management of chronic conditions
- Mental health screening, assessment and counseling
- Substance abuse screening, assessment, and counseling
- Ambulatory detoxification

Fees:

Medicare, Medicaid, private insurance, and self pay. Self-pay patients are charged on a sliding scale fee based on income.

Eligibility:

Open to all residents of Atlantic County, with a focus on homeless individuals.
